New Students

New Student Pre-registration Form

24-25 New Student Registration/Enrollment
August 27th 2024 - June 5th 2025

Once the Pre-Registration Form is complete go to the Gale-Ettrick-Trempealeau Middle School Gale-Ettrick-Trempealeau School District Pupil Services Office Please ask for Karen Smith when you get there. 19650 Prairie Ridge Lane, Galesville, WI 54630. 

  • Please bring Required documentation with you.

  • Students Immunization Record

  • Original Birth Certificate

  • Proof of Residency (Tax Bill, Utility Bill, Drivers License, ect.)

  • School Transfer Information - with correct contact information

  • District Bus Transportation Form (or do in person)

After all is provided, Karen will accept your student into G-E-T School District. The Parent Portal  Access will be created and we will send your account information with the email provided.
It might be up to 1-3 days depending on day of the week submitted.

Then we will need you to sign into this Parent Portal and do all the required forms in the Enrollment Tab and any optional Forms in the General Tab needed.

It Is Important That All Forms Are Filled Out In The Enrollments Tab In The Powerschool Parent Portal Account

Form Instructions

In the form you will enter in the basic information needed to start the new enrollment process.

 

Before you begin your online registration for your new student, please gather the following:

Household information - address along with **Proof of Residency for new student enrollments (unless applying under the open enrollment
application) and phone numbers for all adult household members
Legal Parent/Guardian information - work and cell phone numbers, email addresses
Student information - demographic information, **Original Birth Certificate, **Immunization Record, health/medication information, as well as
previous school report card or transcript as appropriate
Emergency contact - hospital, doctor, dentist and emergency contact phone numbers and addresses
Request for Records - address and phone of previous school unless never before attended school

The documents marked with an *(asterisk) above are required documents to enroll your student(s) and will need to be either uploaded in the application or brought into the Pupil Services office located in the Middle School before the application will be processed. 

Proof of Residency - accepted documents:
- Current Utility Bill (within 45 days) - gas, electric, or water with name, address and current service address
- Current Cell Phone Bill (within 45 days) - with your name and address
- Current Tax bill for your home - with your name and address
- Lease Agreement - current address, names of all parties residing at the address, effective date and all parties signatures

Other Documents needed:
Original Birth Certificate - if you are in need of an original birth certificate for your student you may request a birth certificate from the
Wisconsin Department of Health Services by calling them at (608) 266-1371 or an completing an application on their website at Wisconsin

Immunization Record - available from your healthcare provider or visit the Wisconsin Immunization Registry Website
Request for Records - this form is available inside the application. Please have your student (s) former school address and phone numbers
available to complete the form unless your student has never been in school.

Open Enrollment

Wisconsin's inter-district public school open enrollment program allows parents to apply for their children to attend school districts other than the one in which they live. These are referred to as non-resident districts.


Students who may open enroll:

~ Any student in 5-year-old kindergarten to grade 12.
~ Prekindergarten, early childhood education and 4-year-old kindergarten
~ Only if child's resident district offers the same type of program, and
~ Only if child is eligible for the program in the resident school district.

Parents must:

~ Submit an application to the non-resident school district during the regular application period.
~ A separate application must be submitted for each child.
~ May request specific school or program, but assignment to requested school or program is not guaranteed.
~ May apply to a maximum of three nonresident school districts per school year.

 

The dates for this year's open enrollment are: FEBRUARY 5-APRIL 30, 2024 for the 2024-2025 school year.
The open enrollment application period closes at 4:00 p.m. on APRIL 30, 2024.


Sorry late applications will not be accepted for any reason.


It is recommended that you apply for open enrollment through the online application process. You may also download and print a paper copy of form PI9410 by visiting the Department of Public Instruction website and hand deliver to nonresident district by 4 p.m. on April 30, 2024.
